It can be obtained as a very rare drop from Lodsmok or the Charred Artifact.

To use this ward, and confirm a 100% success rate when upgrading items at the Draconic Forge, first press 'Upgrade' on the forge. Then, select the item you want to upgrade and press 'Empower'. Doing so will show two options in your chatbox.

Select the first option that will say 'Empower using Perfect Upgrade Ward. Then, you will have a 100% success rate on your upgrade.
